A few quick questions and we will get you to the right appointment. This takes about a minute, and it does not book a time. A member of our team will call you to confirm.

New Patient Exam and Cleaning. A full exam, cleaning, the X-rays we need, and an oral cancer screening, plus a clear plan for anything we find.

Dental Emergency. Pain, swelling, a broken tooth, or a filling or crown that came out.

General Consultation. Not sure what you need. Start here and we will work it out together.

Dentures and Partials. New dentures, a partial, a reline, or a repair.

Cosmetic Consultation. Whitening, bonding, or veneers.

Nightguard or Sleep Appliance. Grinding, a sore jaw, or an appliance your physician has prescribed for snoring or sleep apnea.

If something hurts, calling is faster than this form. Call (518) 762-4321 and we will help you judge how quickly you should be seen.

If you have facial swelling that affects your breathing or vision, or bleeding you cannot control, call 911 or go to the nearest emergency room.

If a tooth hurts, something broke, or you are worried about swelling, call the office at (518) 762-4321. We will help you decide how quickly you should be seen and arrange the next available appointment.

If the office is closed, leave a message and we will return calls at the start of the next business day. If you have facial swelling that affects your breathing or vision, uncontrolled bleeding, or a head injury, go to the nearest emergency room or call 911.
